On March 25, 2011, Attorney General Andrew Swan announced that the Honourable Ted Hughes will conduct an Inquiry to examine the circumstances surrounding the death of Phoenix Sinclair and in particular to inquire into:

The findings, conclusions and recommendations to better protect Manitoba children will be forwarded to the Attorney General by March 30, 2012, following which the report will be made public. Commissioner Hughes may also give the Attorney General any interim reports he considers appropriate to address urgent matters.
